In consideration of the undersigned minor child named below (the Participant) being allowed by Laguna Coast Volleyball Club, a California corporation, (LCVC) to participate in the activity identified above (the Activity), the undersigned, on their behalf and that of the Participant (the Releasing Parties), hereby waives, releases and forever discharges LCVC, and its officers, directors, employees, agents and their affiliates, (the Released Parties), from any and all claims, rights, or causes of action, known or unknown, arising from or pertaining to the Participant’s involvement or participation in the Activity, including death, injury, financial loss, loss of property and any and all other damages (the Losses). The Releasing Parties, and specifically the Participant, the acknowledge the risk of the Losses and assume fully responsibility for such Losses occurring or being caused to any extent from their participation in the Activity.

If the event that LCVC is unable to contact the Parent or Guardian upon the occurrence of injury to the Participant, LCVC is authorized to take all steps it deems necessary, in its sole discretion, to obtain medical assistance, the cost of which shall be reimbursed or paid for by the Parent or Guardian.

The Participants also authorize LCVC to take photographs or videos of the Activity; and they waive and release LCVC and the remaining Released Parties from any claims arising from or pertaining the ownership, dissemination or use thereof.

I also authorize LCVC to use photographs and videos of the Participant specifically, other than those taken of the Activity, in the LCVC website and its social media promotions.

Laguna Coast Volleyball Club (LCVC) has implemented preventative measures to reduce the
spread of COVID-19; however, LCVC cannot guarantee that you, your child(ren) and other family
members will not become infected with COVID-19 while being present at participating in LCVC
activities.

The undersigned, by signing this document, agrees and acknowledges on behalf of their LCVC
participant child(ren) identified below, themself and their family members, that they understand
and assume all risks, losses, damages, expenses and health conditions caused by or related to
their any infection with COVID-19 arising from or pertaining to their being present at or
participating in LCVC practices, tournaments, games, travel or other activities.

The undersigned also specifically agrees to comply with LCVC’s Covid-19 preventative measures
and to be solely responsible for the health risks to their participant child(ren) named below,
related to or arising from their participation in the foregoing LCVC activities notwithstanding the
adequacy such Covid-19 preventative measures or the enforcement thereof by LCVC.

The undersigned also agrees to hold harmless and indemnify LCVC, its employees, agents and
Board members, from and against all claims, causes of action, losses, damages, expenses or
liability for injury to any other person or property, arising from or pertaining to their child’s or
children’s participation in LCBC’s activities.
